School of Medicine Undergraduate Medical Education Document Requests

This online request form can be used by current and/or former University of Colorado School of Medicine MD Students (Undergraduate Medical Education). Requests may take up to 7-10 business days after submission. Upon completion, the Student Data Records Manager will send an email notification of confirmation.

Transcripts

An official transcript is a comprehensive record of academic coursework and progress within the University of Colorado system that reflects earned degrees as well as credits accepted in transfer or by exam.

If you only need an official transcript (with no accompanying form or letter), please order the transcript from the Parchment - Office of the Registrar to be mailed directly to the address you wish to receive it.

If an official transcript is needed with a state board form, please order the transcript from Parchment - Office of the Registrar and have it emailed to: deborah.jackson@cuanschutz.edu

Diplomas 

Class of 2013 and subsequent - We have on file a copy of your diploma.

Class of 2012 or Prior - If you require a true copy of your diploma, scan a copy of your medical school diploma and include it with your request. If you do not have a copy of your diploma, you can order a Certified Electronic Diploma.

Certified Electronic Diplomas

CU Anschutz recently implemented CeDiplomas (Certified Electronic Diploma).

Please note: This service is only available to students who graduated as of Spring 2022 and later. For students who graduated prior to this, please contact the CU Anschutz Office of the Registrar at 303.724.8000 or Registrar@CUAnschutz.edu.

Once you’ve ordered and downloaded your CeDiploma, it can be shared indefinitely with family, friends, employers, and social media. Each CeDiploma also comes with a unique 12-digit certified electronic document identifier (CeDiD), which provides the ability for the electronic diploma to be validated on CU Anschutz’s website. You will receive an email with a link to download your credential when it is available from our trusted partner, CeCredentialTrust. When you download it, it will take you directly to Paypal for a one-time payment ($15.95) and then it will become available for you.

Medical Education Verifications

Undergraduate Medical Education (UME) Verifications (Doctor of Medicine/Medical Students)

State Licensure Applications - Most states have a form for degree verification. Complete the parts of the form that ask for your personal information, including your signature (if required), then attach the form to your request. We will complete it and remit it directly to the State Medical Board. 

For all California Medical Board

For all California Medical Board verifications please attach the MED form to your request. You will also need to order an official transcript from Parchment - Office of the Registrar and have it emailed to: deborah.jackson@cuanschutz.eduOnce both the MED form as well as the transcript are received, they will be uploaded with a copy of your diploma to the Medical Board of California DOCS portal. A confirmation email will be sent to the email address on the request form once complete. 

Graduate Medical Education Verifications (GME) (Residency/Fellowship)

Residency and Fellowship requests are conducted through the GME department. If you require Graduate Medical Education Verifications for Residency and/or Fellowship training, please visit the GME site for additional information.

Click the link above, navigate to Training Verification, select Program Coordinator, go to the specific department, and select the appropriate program coordinator to obtain the verification from, and reach out to them.

Medical Student Performance Evaluations (MSPE/Dean's Letters)

The Medical Student Performance Evaluation (MSPE), formerly known as the Dean's Letter, is an official medical school document. The MSPE is ONLY allowed to be used for residency and fellowship application purposes; students may not possess a copy of the document themselves. If you require a copy of your MSPE for a MIDUS fellowship application, please remit your request here. If you are not applying through MIDUS, CU Anschutz can send it directly to any program requesting a copy via USPS mail or email. Please remit your request through the form. 

All MSPE letters will be emailed as an encrypted PDF File. It also serves as an attestation that: "the undersigned, declares under penalty of perjury (under the policies of the University of Colorado School of Medicine) that the foregoing is true and correct and that you are the individual stated. In the event of identity theft or false representation, the individual shall be prosecuted to the full extent of the law".

ERAS Residency Token Request

The ERAS Residency token is a fourteen-digit alpha-numeric code that is sent to you via email. You need this token to register at the MyERAS website to begin the application process. Detailed login instructions will be included in your token email. Simply follow the link to request a CU Anschutz token.

You will receive a token via email once the token request has been processed. If you do not receive your electronic token, check your Junk mail; otherwise, send an email to som-student.records@ucdenver.edu. Once you have received your token, copy the token code and use it to register at MyERAS. You only need one (1) electronic token per season. You can use your token to apply to as many specialties as you want.

MIDUS Fellowship MSPE Dean's Letter and Transcript Request

If you are a School of Medicine alumni applying for a fellowship through ERAS, the Student Records Manager can upload your transcript and Medical Student Performance Evaluation (MSPE) directly to the ERAS Fellowships Documents Office (EFDO) through the Medical Institution Document Upload Service (MIDUS). 

The EFDO Medical Institution Document Upload Service (MIDUS) provides the University of Colorado School of Medicine (CUSOM) with the ability to upload the Medical Student Performance Evaluation (MSPE) and medical school transcript directly to the EFDO on behalf of an applicant. In order for CUSOM to upload a document on your behalf using MIDUS, you must send an electronic request to CUSOM using EFDO Online Services. Please remit two (2) requests if you need both the MSPE/Dean's Letter and a transcript uploaded.

Documents submitted through MIDUS are expected to take up to five (5) business days to process. MIDUS Requests in EFDO. Please use the following contact information when submitting your MIDUS request:

Once you submit your request to the EFDO Online Services, our office will receive an email with a link to access MIDUS and upload instructions.

NBME Certification of Identity

You must be officially enrolled at CUSOM in order to submit an application and/or take a USMLE exam. If you are dismissed, withdrawn, or suspended from medical school, you are not eligible for USMLE, even if you are appealing or contesting your status.

Registration Instructions - Obtaining a USMLE ID number

Visit https://apps.nbme.org/nlesweb/#/login. Click first-time user, and enter your name, SSN, and medical school information. It is important that you enter your name exactly how it appears on the identification that you will present on the day you take the exam. (i.e.; driver’s license, passport).

The NBME will e-mail you your USMLE ID and a temporary password. This may take up to one business day. Please keep your USMLE ID number as you will need it to register for future exams.

Application Process

Once you receive your USMLE information, you should log in to the menu item: Apply for the USMLE. You will be required to change your password at this time. You will then be successfully logged on to the NBME Interactive Website. To start a new application, click on “NEW”. You will be asked to read the application instructions. Then select “next”. The examination you are applying for is Step 1. Follow the instructions and complete the application.

Registration:

Go to https://apps.nbme.org/nlesweb/#/login. Continue through each page completing requests for information (see NBME Sample Online Application) Print the Certification of ID form and make payment for the exam.
